How much does GEO for a dental clinic cost in 2026?
For an independent practice or a small group, roughly $1,000 to $8,000 a month, depending on scope. WebFX puts small-business generative engine optimization (getting a business named inside AI answers) at $1,500 to $5,000 a month. Digital Elevator's pricing guide places a monitor-and-maintain retainer at $1,000 to $2,500 and mid-market work at $2,000 to $8,000. Larger companies pay well above that, up to $25,000 and beyond.
Both guides were published in May 2026, and both are agency-authored, so read them as the going rate rather than an independent survey. They agree on the shape of the market: there is a tool tier, a template tier, and a tier where someone is doing the work.

Below about $1,000 a month you are usually buying software, not an agency. Digital Elevator is direct about what sits in the $500 to $2,500 productized band: template-based work with limited customization. That can be fine if you have someone in-house to run it. Annual billing takes 15% off the retainer and is paid upfront; setup fees are one-time and are not discounted. Monthly plans have no lock-in and no notice period, so you can cancel any day and billing stops immediately — a month cancelled before its work has started is refunded in full, and if its work has started we finish the month you paid for. The setup fee is non-refundable once setup work has begun, except under the 45-day citation guarantee, which hands it back if you have not been named by at least two of the four engines in an agreed question within 45 days of setup being confirmed complete. Availability is capped rather than open-ended: at most three cosmetic or Invisalign practices per US state at any one time, because every account is run personally by the founder rather than handed to an account manager. When a state reaches three it closes to new clients until a slot opens. On Dominance that tightens into contractual metro exclusivity — one practice per metropolitan area for as long as the subscription is active.
What does "3,600 AI answers a month" actually mean?
30 questions × 4 engines × 30 days. The same set of patient-style questions is asked on ChatGPT, Perplexity, Gemini and Google AI Overviews every day, and what you get back is the pattern rather than one lucky result. Authority raises it to 45 questions and 5,400 answers; Dominance to 60 questions and 7,200 answers, with competitor questions included.
Daily is the point, not the padding. Profound tested roughly 80,000 prompts per engine and found that about 40 to 60% of the domains cited in AI responses are different a month later. Ask once and you have a snapshot that may not survive the week. Ask the same question 30 times in a month and you can tell a real change from noise, and see which competitor keeps taking the slot.

Most of that list happens away from your website, and the correlation data is why. Across 75,000 brands, Ahrefs measured mentions of a brand elsewhere at 0.664 against 0.218 for links pointing at its own site. In a second study of the same size, YouTube mentions came out highest of all at about 0.737. A retainer spent entirely on your own pages is spending against the evidence. What is not included, and what we will not promise
No guarantee of rankings, citations, patient numbers or revenue. The recommendation itself is not for sale. Four engines are fully tracked: ChatGPT, Perplexity, Gemini and Google AI Overviews. - No outcome guarantee. Citevio does not guarantee rankings, citation positions, patient volume or revenue. Anyone who does is describing something these systems do not do. The 45-day citation guarantee is not an exception to this: it promises a refund of the setup fee, not a result.
- No paid placement. Ads are appearing around AI answers, but the recommendation inside the answer is assembled from public signals. It can be earned, not bought.
- Four engines, not six. ChatGPT, Perplexity, Gemini and Google AI Overviews are covered. Claude and Grok visibility is in pilot measurement; we publish the numbers to this plan once they are sufficiently validated.
- No bought or filtered reviews. Every patient gets invited, nobody is screened out first, and nothing is incentivized. That is what keeps the Review Engine safe under the FTC's fake-review rule.

How long before anything moves
No agency can guarantee that an AI engine will name a practice, and Citevio does not offer that guarantee. AI placement cannot be bought; there is no paid slot in an AI recommendation.
Does the arithmetic work for an independent practice?
A single Invisalign case typically runs $5,000 to $8,000; three to four veneers, $2,775 to $10,000 — national averages that move with your market and the case itself. Authority at $2,900 a month is $34,800 a year plus a one-time $3,500 setup. At $5,000 a case, that is roughly eight cases in the first year and about seven in the years after. That is arithmetic, not a forecast.

The reason the arithmetic is worth doing at all is that the baseline is low for almost everyone. SOCi's 2026 Local Visibility Index looked at nearly 350,000 locations across 2,751 brands and found 1.2% recommended by ChatGPT, 7.4% by Perplexity and 11% by Gemini, against 35.9% appearing in Google's local 3-pack. Being absent from AI answers is the normal condition right now, which is exactly why it is still cheap to fix.

Our own scans point the same way and are narrower on purpose. In June 2026 we scanned 154 Charlotte dental practices and 58 in Columbus. ChatGPT named two of them in total. The Charlotte practices it skipped were not weak by classic standards: they averaged 4.82 stars, and 147 of the 154 had sites fully open to AI crawlers. Why publish a price at all?
Because "contact us for a quote" gives an AI assistant nothing to quote. When a clinic owner asks an assistant what this kind of work costs, the pages that answer with a number are the ones that can be repeated back. That is our own reading of how these engines handle buying questions, and this page is us acting on it.
The second reason is the one that binds us. A published price is a commitment you can be held to. It makes a like-for-like comparison possible without a sales call, and it means the number you see is the number, not an opening position that depends on how your practice looks on paper. If the plans change, the home page is the list that gets updated first: current pricing lives there.
If you stop paying, what stops and what stays?
What was changed on your website is yours and stays where it is: the pages, the structured data, the profile corrections, the question set and the raw engine answers collected along the way. What stops is the measurement and the work that follows it. In plain terms, you stop finding out what is happening rather than losing something you own.
The billing side of leaving is answered in the contract questions further down this page — no lock-in on monthly plans, and what happens to an annual term or the setup fee. This section is about the other half of that question, which contracts do not cover: what happens to the visibility itself.
Here is the honest answer, and it is shorter than a sales page would like. We do not know. Citevio has never tracked a practice's engine visibility after an engagement ended, because that measurement has not been run. Anyone telling you that your visibility decays on a particular curve after you stop is describing a fear, not a finding, and the correct response is to ask them for the study.
What can be said comes from measuring the answers while the work is still running. Across three fixed queries run twice on each of two engines through their APIs on the evening of 15 August 2026, 12 of 12 calls completed, and the displayed order moved between runs — on one engine the same query returned first place in one run and third in the next, which is one evening's photograph rather than a trend. If the order moves that much inside a single evening with the work in progress, no one is in a position to publish a decay rate for what happens after it stops.

That volatility is the general condition rather than something particular to us, and it is written up with the sampling behind it in how often do AI answers change. How do you prove an AI citation turned into a patient?
AI answers increasingly resolve without a click — that's exactly why Layer 5 of the protocol measures what happens after the answer, not the click itself. It adds a source field and an "AI assistant" option to the booking form, a call-tracking template, and an "AI-sourced" line in the monthly report; the appointment data stays in the practice's own systems, only the monthly count reaches us. This gets installed with a practice's actual booking form at onboarding — until then, it's a design commitment, not a delivered number.
Might this budget work better somewhere else?
